 We have provided the list of websites below to provide additional resources for learning more about person-centered and patient-centered programs, services and organizations. Person-centered planning is a process-oriented approach to empowering people with disabilities. It focuses on the people and their needs by putting them in charge of defining the direction for their lives.

The Commonwealth Fund is a private foundation that aims to promote a high performing health care system that achieves better access, improved quality, and greater efficiency, particularly for society's most vulnerable, including low-income people, the uninsured, minority Americans, young children, and elderly adults. The Fund carries out this mandate by supporting independent research on health care issues and making grants to improve health care practice and policy.

The National Health Council is implementing an Integrated Patient-Centered Care (IPCC) Initiative within its programming and activities. The vision of the IPCC model is a health care system integrated across time, place and profession to maximize value by delivering patient-centered care throughout a person's lifetime.

The American College of Physicians has proposed a bold solution to what they say is a looming collapse of primary care medicine in the United States: a patient-centered health care system. Patient-centered health care is a system that builds upon the relationship between patients and their primary and principal care physicians and supports the systems needed to achieve better results.


Employment and Disability Institute, Cornell University has produced this website with funding by a grant from the Developmental Disabilities Planning Council, to complement a three year training and technical assistance grant project begun in 1998 which has provided training and support to nine demonstration sites throughout New York State that are integrating person-centered planning into transition planning and services for students with developmental disabilities so that they might become active participants in the shaping and realization of their future as adults.
